In an effort to make it easier and faster for veterans to receive their disability benefits, the VA is standardizing current forms and creating new ones. The VA Form 21-4138, or the Statement in Support of Claim, is one of the forms with new changes.
What purpose does the VA Form 21-4138 serve?
In summary, this form is used to provide information to corroborate a veteran’s claim for benefits. This form is filled out by a veteran’s friends, families, or former service members. Those filling out this form provide statements supporting the veteran’s claim and verifying the limitations the veteran suffers because of their disability. Former service members who served alongside the veteran can submit firsthand accounts of the in-service incident that led to their current disability.
What has the VA changed this form to be now?
In 2021, the VA replaced the VA Form 21-4138 with five new forms. These five new forms will help veterans better communicate specific requests to the VA, and even expedite the claims process. The five new forms are:
- FOIA/Privacy Act Request – VA Form 20-10206
- Priority Processing Request – VA Form 20-10207
- Document Evidence Submission – VA Form 20-10208
- Lay/Witness Statement – VA Form 21-10210
- Chapter 31 Request for Assistance – VA Form 28-10212
